For a contractor this looks like: controlled RAMS and ITPs, inspection and test records that match the ITP, subcontractor approval and review, calibrated equipment, snag and NCR tracking, and handover files a client can rely on.
We take organisations to the point where an accredited certification body can assess them with confidence: gap analysis → implementation → operating evidence → internal audit and management review → readiness review. The assessment and the certificate belong to the certification body; our role ends at prepared-and-ready, and continues afterwards only as support.

Not legally, but many PQQs and frameworks ask for it or for an equivalent documented system. Certification is the simplest way to answer the question once rather than per tender.
For a small contractor with engaged management, typically 3–6 months to operating the system, then a certification body performs a two-stage assessment. Timescales depend on how much of your existing practice already fits.
No. Templates are a starting point; conformity comes from operating the system and being able to evidence it. A competent person must adapt, approve and embed every document before it is relied on.
