  1. Status of actions from previous reviews.
  2. Changes in internal and external issues, interested parties and legal obligations.
  3. Performance: objectives, KPIs, audit results, nonconformities and corrective actions.
  4. Incidents, near-misses, complaints and customer/stakeholder feedback.
  5. Resource adequacy: people, competence, plant, budget.
  6. Effectiveness of risk and opportunity actions.
  7. Improvement opportunities and decisions — each with owner and date.
  8. Minutes rule: every decision has an owner and a date, and appears in the next review’s first item.
